A tattoo is a controlled micro-injury
When a tattoo artist deposits ink into the skin, they create a series of micro-perforations with fine needles that place the pigments in the dermis, the middle layer of the skin.
- The needle passes through the epidermis to reach the dermis.
- It deposits droplets of ink there.
- This triggers an immediate inflammatory response: your immune system treats the pigments as intruders.
The process is natural. That initial inflammation is in fact essential to start the healing process.
Phase 1: Inflammation (days 1 to 3)
Straight after the tattoo, the skin enters a phase of acute immune reaction:
- It becomes red, warm and slightly swollen.
- White blood cells, such as macrophages, are mobilised to “clean up” the foreign bodies.
- You may notice some weeping (a mix of lymph, plasma and ink residue).
This phase is comparable to a superficial wound and marks the start of tissue repair.
Phase 2: Regeneration (days 4 to 14)
During this period:
- The epidermis rebuilds itself at the surface to protect the tattooed dermis.
- A thin crust or film appears, and it must never be picked.
- Itching can occur, which suggests the skin is healing well.

Phase 3: Stabilisation (weeks 2 to 6)

- The immune system stops attacking them.
- The colour stabilises and the tattoo takes on its final appearance.
This stabilisation is also what makes tattoo removal more complex: the ink is deeply anchored and needs targeted laser pulses to be broken up without damaging the tissue.
Why does the ink not disappear?
Biology is well designed: when white blood cells cannot clear the pigments, the body “accepts” them and traps them in immobile cells. That is what allows a tattoo to stay visible over the years, even though some particles can migrate slightly or fade with time.
What can change the skin’s reaction
Your body initially tries to eliminate the pigments like any intruder. But ink particles are too large to be cleared completely.
- Macrophages “swallow” the pigments but cannot remove them all.
- The body therefore ends up encapsulating them in immobile cells.

Does my body try to reject the ink?
Yes, that is exactly what happens at first. When you get a tattoo, the body treats the ink pigments as foreign bodies. Immune cells, particularly macrophages, are immediately sent to the site to neutralise them. However, as the ink particles are too large to be fully digested or eliminated, they remain trapped in the dermis. The body then "tolerates" them and encloses them within fixed cells, which makes the tattoo permanent, until an external intervention such as laser tattoo removal.
Why does my skin peel after a tattoo?
Skin peeling is a completely normal reaction after a tattoo. It is the renewal of the epidermis, the skin's surface layer, which was pierced by the needles during the session. As new skin forms, the old dead skin comes away, creating a peeling effect comparable to sunburn. This process contributes to proper healing of the tattoo. It is essential not to scratch or pick at the scabs, as this could alter the final result of the tattoo or cause scarring.
Can a tattoo fade?
A tattoo is permanent, but it can fade over time.
